1. Program Overview<\/h2>\n

The HST Device Advancement Program<\/strong> offers eligible business customers the opportunity to receive a one-time discount code for purchasing Belun\u00ae Ring (s) via authorized Belun channels, such as the Belun e-shop.<\/p>\n

Participants submit information and serial numbers of their qualifying HST devices through Belun\u2019s designated online form for eligibility review. Following Belun\u2019s verification and approval, the participant will be invited to execute a Participation Agreement to finalize program enrollment and receive the applicable discount code. No device return or collection is required.<\/p>\n

2. Device Qualification and Verification<\/h2>\n

2.1 Approval is based solely on the information and photos provided by the Participant during the submission process.<\/p>\n

2.2 Belun may request additional documentation to verify device ownership and authenticity.<\/p>\n

2.3 Only devices meeting the Program\u2019s eligibility criteria will be reviewed. Submission does not guarantee approval.<\/p>\n

2.4 Belun is not responsible for any device shipped, stored, handled, or provided by the Participant, and Participants acknowledge that the Program does not require shipment or transfer of physical devices unless separately requested in writing.<\/p>\n

3. Eligibility<\/strong><\/h2>\n

3.1 Participation is limited to new Belun clients, with legally formed clinic business entities located in the United States.<\/p>\n

3.2 Each business address may submit a maximum of ten (10) eligible devices.<\/p>\n

3.3 Devices must be fully owned by the Participant and not encumbered by liens, leases, financing agreements, or third-party claims.<\/p>\n

3.4 Eligible Device Criteria: Any FDA-cleared HST device marketed in the United States. The submitted devices are obsolete or not in use by the participants.<\/p>\n

3.5 Required submission information includes, but is not limited to: brand name, device serial number(s), quantity, and device photographs.<\/p>\n

3.6 Devices that do not meet eligibility criteria will not qualify for a coupon.<\/p>\n

3.7 Duplicate or repeated submissions of the same device will be rejected.<\/p>\n

3.8 Participation in the Program is voluntary, and the Program is not a sale, buy-back, device-replacement, or trade-in program under any state law. It is solely a promotional credit program.<\/p>\n

4. Promotional Credit<\/h2>\n

4.1 Each approved device qualifies for one (1) USD $50 valued discount toward the purchase of a \u500d\u9748\u6307\u74b0<\/strong>. Each Participant qualifies for up to ten (10) devices submission.<\/p>\n

4.2 The Belun\u00ae Ring requires the separate purchase of the Belun Online Report Subscription service. The subscription is not included in and cannot be purchased using the coupon(s).<\/p>\n

4.3 Coupon codes will be sent within fifteen (15) business days after Belun receives the fully signed Agreement.<\/p>\n

4.4 Coupons are non-transferable, non-refundable, not redeemable for cash or cash equivalents<\/strong>, and valid only for direct purchases made through the Belun e-Shop on or before Jan 31st, 2026<\/strong>.<\/p>\n

4.5 Coupons may not be combined with other promotions, discounts, or volume agreements unless authorized in writing.<\/p>\n

4.6 Coupons do not constitute gift certificates, prepaid cards, stored-value cards, rebates, or any instrument regulated under state gift-card or escheat\/unclaimed-property laws.<\/p>\n

4.7 Belun may refuse to honor coupons obtained or submitted fraudulently or in violation of this Agreement.<\/p>\n

4.8 The Program is offered on a limited-availability basis and will be closed when all discount codes are earned. Upon depletion of the allotted codes, the Program will terminate without further notice.<\/p>\n

5. Ownership Representation<\/h2>\n

The Participant represents and warrants that all submitted devices are owned exclusively by the Participant and are free of liens, leases, or third-party ownership or security interests. Participant agrees to indemnify Belun for any claims arising from inaccurate ownership representations.<\/p>\n

6. Program Changes<\/h2>\n

6.1 Belun may modify, suspend, or terminate the Program at any time.<\/p>\n

6.2 Belun may also reject, suspend, or terminate participation if the Client submits inaccurate, duplicated, altered, or fraudulent information\u2014including false serial numbers, modified photos, or repeated invalid submissions. Belun may request additional verification and is not obligated to process claims deemed suspicious or unverifiable.<\/p>\n

6.3 Any modifications, updates, or announcements regarding Program closure will be posted on the Program\u2019s Terms & Conditions and FAQ webpage.<\/p>\n

7. Fraud, Misrepresentation, and Abuse of Program<\/h2>\n

7.1 Fraudulent actions\u2014including falsified serial numbers, altered photos, repeated submissions of the same device, or misrepresentation of eligibility\u2014constitute a material breach of this Agreement.<\/p>\n

7.2 Belun may immediately suspend or terminate participation, deny pending or future requests, and invoice the Client for any resulting losses or replacement costs. 7.3 Belun may also report fraudulent behavior to authorities and pursue legal remedies.<\/p>\n

8. Remedies for Fraud or Misrepresentation<\/h2>\n

If Belun determines that the Client engaged in fraud, misrepresentation, or Program abuse, Belun may, at its sole discretion:<\/p>\n

i.\u00a0 \u00a0 \u00a0Require the Client to return any Ring(s) obtained under the Program\u2014without refund, within seven (7) days of notice; and\/or<\/p>\n

ii.\u00a0 \u00a0 Invoice the Client for the value of any discounts, coupons, special pricing, or benefits improperly obtained, which shall be immediately due upon invoice.<\/p>\n

Failure to return the Ring(s) or pay the invoiced amount constitutes an additional breach, and Belun may pursue all legal or equitable remedies, including recovery of device replacement costs and associated fees.<\/p>\n

9. Limitation of Liability<\/h2>\n

9.1 To the maximum extent permitted by applicable law, Belun is not liable for indirect, incidental, special, punitive, or consequential damages arising from or relating to this Program.<\/p>\n

9.2 Belun\u2019s total aggregate liability under this Agreement shall not exceed the total value of coupons issued to the Participant.<\/p>\n

9.3 The Program is provided \u201cas-is,\u201d without warranties of any kind, express or implied.<\/p>\n

10. Compliance With Law<\/h2>\n

10.1 Participant shall comply with all applicable federal and state laws governing promotions, advertising, and device-related representations.<\/p>\n

10.2 Nothing in this Agreement shall be construed as a resale arrangement, gift certificate \u201ctrade-in program, rebate program, or service contract under any state regulatory framework.<\/p>\n

10.3 Coupons issued under this Program have no cash value<\/strong> and are not redeemable for cash in any jurisdiction.<\/p>\n

10.4 This Program does not provide remuneration intended to induce referrals under federal or state healthcare laws.<\/p>\n
